Complicated: In order for your insurance company to pay for the test and approve it, there needs to be a reason. And a good one at that. Most insurers will pay that code

Misdiagnosis: In order to have a diagnosis of threatened abortion you have to experience bleeding with pregnancy and have had an exam showing that your cervix is still closed and pregnancy still viable. The ultrasound was done maybe because there was a concern that you might have a miscarriage or a "threatened abortion."
