Cyst: Most cancers can be hypoechoic but anechoic is a cyst. In the liver cysts can contain "stuff" that lead to echoes. A CT with x-Ray contrast will be important to prove it is a cyst . By the way, MRI is also excellent for proving cysts

Most incidental: liver lesions found on ultrasound are benign, but sometimes the ultrasound appearance is not specific and CT or MRI is necessary for confirmation. If CT or MRI is not conclusive for a benign lesion, then short interval follow up or needle biopsy may be indicated.
