When under pain management for 2yrs using butrans patch and there is a need for surgery how would the patient handle pain mgmnt with the surgeon?

Worry re: withdrawl!: If the B. patch is taken off and no narcotics are given, the pt. will likely suffer withdrawal symptoms. The patch can be continued and the anesthesiologist can over ride the buprenorphine but must be careful to maintain respiration. After surgery the narcotics can be weaned of and the pt. can resume the bup. If patch is taken off, the pt. must be opioid free before re-starting the patch.
