Cystadenoma: This size lesion expanding to stomach can not be the classic adenoCa. It is probably an islet cell neoplasm or more likely cystadenoma. Since the latter almost always converts to carcinoma, resection of distal pancreas, spenectomy since splenic vein always involved and partial gastectomy must be performed. Laparotomy should be used and depending on path, chemo may be needed PO.
