Several ways: There are a number of features that can be used to distinguish acute from chronic venous thrombosis. In acute clot the lumen of the vein will be hypoechoic. The vessel will be enlarged and there may be peri-venous edema. I chronic clot the intraluminal echoes will be hyperchoic. There may be evidence of recanalization. The vein may be contracted.
