Exact: mechanism not known but drugs that affect nirepinephrine do help pain. Suggest Savella (milnacipran) ot Cymbalta

Neurotransmitters: This is a very complex question, and you need to discuss this in detail with your treating physicians. Both Norepinephrine and serotonin act on the dorsal horns of the spinal cord to inhibit excessive transmission of nerve cell fibers, thereby decreasing the central (brain) perception of pain. Nortriptyline should not worsen that, and may help some. The SNRI's medications help with pain relief.
