IV heparin: IV heparin can be given for multiple reasons such as MI ( heart attack), DVT, Pulmonary Embolism, Atrial Fibrillation, Mechanical heart valves, Other thromboembolic diseases. In certain situations patient are on outpatient anticoagulants which may need to be stopped during preop and IV heparin is initiated. Unsure what the reasoning in this situation is. Hope this helps.
