Under MD supervision: Weaning of opioid medication should be done under medical supervision. Every tapering regimen is individualized to each patient in order to avoid withdrawal symptoms. In addition, your doctor may start you on different non-opioid medications (Gabapentin, Cymbalta, (duloxetine) etc) to help control the pain. I would recommend to ask your doctor who knows your history about weaning off your medications

Understand, but: This will not be a simple taper, as will literally take weeks or months. Suboxone may be needed to do this properly. Find an experienced pain management specialist. arrange a plan and work closely with the doctor.
