Prostate? Stricture?: Such symptoms could be from prostatitis or something partially blocking urine in the urethra, e.g. a stricture (often due to scarring from past infections, especially gonorrhea) or a growth like a wart or fibroma. Non-response to antibiotics fits with all these, and is evidence against an easily treated infection. See a urologist or one of UK's excellent NHS GUM clinics.
