I've read enlarged spleen from fatty liver only occurs after extensive liver damage.can a "mildly fatty liver" cause a painful enlarged spleen?

Shouldn't: Cirrhosis causes splenomegaly through portal hypertension and splenic congestion; a mildly fatty liver should be well before this stage, and should not be the cause of a painful enlarged spleen. You should see a doctor for further evaluation and testing.
