See a dentist: The ability to stick ones tongue out of their mouth varies from individual to individual. This may be your normal or a result of some problem. The two things that concern me are your loose tooth and a swelling. It would be very wise for you to see a dentist asap to see what's going on. If you have a problem and avoid getting it seen and treated, you'll probably have a much bigger problem soon!

Seek evaluation: It sounds like you have a tooth or gum infection that is spreading into the tissue near your tongue. If that is the case you will need antibiotics asap. You may want to schedule a dentist appointment right away.
