Can all blood work (comp. meto panel, cbc, etc) all be normal and still have cirrhosis?

Maybe, but...: No system of the body is spared with end-stage liver disease. Will see general deterioration, endocrine problems, cardiovascular, GI (diarrhea, gastropathy) pancreatic-biliary, neurologic, hematologic (coagulopathy; bone marrow abnormalities), lung (hepatopulmonary), kidney (hepatorenal), lymphatic, dermatologic, musculoskeletal, higher risks of cancer. Other testing & exam will detect these.

Yes, it is only when cirrhosis gets complicated with portal hypertension and splenomegaly when cell counts, including platelets may be affected. In early stages, cell counts remain normal.

Not exactly. A person with cirrhosis has advanced liver disease and multiple laboratory parameters are abnormal, including liver enzymes, low serum albumin, polyclonal hypergammaglobulinemia, and hormonal imbalance.
