MRI : The two most likely causes are an adhesive capsulitis (frozen shoulder), or a deeper problem with the shoulder itself such as a rotator cuff tear. So if it did not respond to PT, then an MRI is done to evaluate further. If MRI negative, an arthroscopic lysis of adhesions is usually what takes care of it. If MRI positive, then that underlying condition is addressed.
