Mri says i have "mild spondylotic changes" at l4/5. is that a fancy way of saying "herniated disc"? are there excercises i should do to fix it??

No: Spondylotic is a fancy way to say degenerative. The disc is worn out. Exercise will not make it better. Herniated disc means different things to different people but the pathologic definition is protrusion of the nucleus pulposis through the posterior longitudinal ligament.
