is premed needed before a dental cleaning if you had a heart ablation procedure in the last three months?

I would: not well addressed in the guidelines, but there is some endothelial damage necessary for the ablation - which heals over time - prophylaxis for the three or six months post procedure is probably over-safe, but the "cost" of prophylaxis is generally quite small.
