How does spondylitis typically show on radiography early on? are there distinct lesions, or could it appear the same as normal degenerative changes?

Very subtle: In its earliest stages, it can be difficult to differentiate the X-ray findings of degenerative arthritis and spondylitis. Spondylitis is more of a systemic condition, so the findings are more likely to be symmetric and multilevel. Spondylitis can involve calcification of the soft tissues, where degenerative arthritis does not.
