Acne: Nipples have fairly large oil glands so can develop blocked pores like blackheads. Open sores sound like the areas are being scratched. A gentle hydrocortisone 1% cream could be tried for the itching. If that resolves, the open sores likely will resolve. There are other possible causes, like impetigo (bacterial infection of skin), Herpes infection, etc. See a Doc if persists.
