Weakness: If the pain does not improve by 2 weeks, then he should be reevaluated more extensively. Moreso if he notes weakness as well, then he should be evaluated immediately.

Need a diagnosis: Tight muscle is not medical diagnosis. The doctor is describing entrapment (nerve trapped between a muscle and something else). This can be identified on a high quality MRI.
