Weak spot: It means the area of the heart muscle is not moving in a correct manner. The most common reason is because the area has been damaged like from a heart attack. However some people have structural heart problems that can cause the area to not move right.

Description: It is a description of what is seen on the echo. the septum is the muscle wall between the left and right ventricles. There are many reasons for this to be abnormal the most common ones are due to ventricular hypertrophy or after a heart attack.
