Surgery.: I'd try a cortisone type ointment, but if that doesn't work, surgery, or banding, depending on type, internal of external.

See your doctor: Luckily most hemorrhoids don't need surgery. Often times, they can be treated in the office with simple procedures very effectively and without pain.
