Sciatica : Most patients with sciatica obtain relief with conservative therapy. If physical therapy with stretching exercises did not help you may need further testing with electromyography and neuroimaging, (lumbar x rays and mri). This will clarify the etiology of the sciatica. This could be due to a disc herniation, or spondylolisthesis (developmental vertebral condition). I wish you a prompt recovery.

Maybe Surgery: It sounds like you have done a lot of good non-operative treatment options. If you are still not getting better after several weeks, it may be time to consult a neurosurgeon to determine if microsurgery would be appropriate.

Possibly not inflamm: Assuming your pain is as a result of a pinched nerve, three causes of pain is inflammation, compression of nerve and nerve injury. If treatments what you had ( for inflammation) do not work then likely need epidural injection or physical therapy. If that fails then surgery.
