Mai lung disease is treated with ethambutal, azithromycin, rifampin. can treatment stop after one year? the patient (87 years) cannot produce sputum.

Not always: Most docs recommend treatment for at least one year after sputum cultures are negative. He may need a lung aspirate culture to confirm its negative if he can't produce sputum. This definitely requires consultation with the prescribing doc.
