If a patient only has one lung, then most surgical treatment options are not feasible or safe. Almost all surgical procedures on the lung use “single lung ventilation” during the procedure, meaning that the opposite lung only is ventilated during the procedure and the lung being operated on is not ventilated. However, if the patient only has one lung, there is no opposite lung to ventilate.

The surgical decision changes that means removing tumor with the minimum clear margin becomes the choice. If the tumor is beyond 2 cms then take advise of your medical and radiation oncologist as some of the newer treatments with immune therapy some patients are getting complete responses or reasonable partial reduction in size and then field of radiation can be decreased and minimum surgery
