Core biopsy of 4 mm lesion. path report said each core was 8-9mm. do they also take out surrounding tissue also? do they know to not just look at that

Yes: It is not unusual for a breast biopsy to have normal surrounding tissue especially for a small sized lesion such as this (4mm). The important thing is the radiologist is confident they sampled the lesion and that what the pathologist sees is concordinant with the radiologist's impression and target.
