Have biopsy.: It is unlikely to be cancer at your age, but you should have the polyp biopsied and removed. This will likely help with the heavy bleeding and give an answer about cancer. It is a minor procedure that can be done in the office sometimes, but likely an outpatient surgery. The cyst can be followed on ultrasound and may be normal function of the ovary, but if it grows or changes may removal.

Biopsy: If your physician hasn't already done an endometrial biopsy or hysteroscopy with d&c, he or she needs to do so. Ultrasound alone can't rule out pre-cancer or cancer of the uterus, it simply reveals polyps, fibroids or ovarian cysts. If the biopsy is normal, then you may elect to try oc's, lysteda, or undergo an endometrial ablation for the treatment of your abnormal bleeding.
