Can be either: Pulmonary hypertension can be caused by disease of the heart such as mitral valve disease, aortic valve disease, holes in the heart and blockages to name a few. But pulm HTN can also be caused by disease of the lung tissue like pulmonary fibrosis or by sleep apnea, too. Your doctor should be abel to help you sort this out.

Dual workup: Typically you should see both a cardiologist and pulmonologist individually to work this up since they will often take different but complimentary approaches.
