Anxiety: Ocd is an anxiety disorder and often the underlying anxiety takes forms other than the ocd symptoms proper. But it's important that your psychiatrist know about these symptoms, especially if you are on medication.

OCD, BFRBs, Tics: It's always good 2 invlove a good detective 2 sort out everything that's going on. In this case consult a psychiatrist or clinical psychologist. Lip-picking & other body-focussed repetitive behaviors like it (nail biting/hair pulling/skin picking) r not usually assoc w/obsessive thoughts. Nor are tics. Bfrbs & tics respond 2 somewhat different interventions than ocd, though there is some overlap.
