Esophageal spasm.: Esophageal spasm or dysmotility. Several drugs can help. You can have several diseases that disrupt contractions in the lower two thirds of you esophagus. Also, scarring from heart burm could have diminished the lumen size. Check with your pcp first, but you will probably need referral for endoscopy!

Check for dysphagia: Since your symptoms are quite severe, i would suggest seeing doc for eval--possible scope is needed. You may just have globus pharyngeous, a common "lump-in-throat sensation" giving a choking sensation w/o actual swallowing difficulty. Common causes: cricopharyngeal spasm, reflux, esophageal dysmotilty, or often from anxiety. This can be bothersome/scary for some. Consult doc. Good luck.
