Sciatica: Pyriformis syndrome stems from the buttock, endometriosus from the pelvis in most cases. Doing a cortisone block at l5, s1 usually will not help.

Cortisone: Possibly. Understand that an injection directly to the irritated area often helps very quickly. Cortisone (depomedrol, actually) is a delayed release medication that will eventually find its way info your bloodstream. So, even a shot in the butt or shoulder would possibly help a reonte area after a delay of a day to a week.
