Should the cortisone shot be in the joint eg l4/l5, that was indicated as the problem. would there be a reason for injecting into l5/s1 ? thanks

Yes: It depends what the MRI has shown. If the pain management doctor suspects that l5-s1 is a contributing factor, then it is prudent to inject both areas to one time rather than risk having you need to return for a second injection.
