See your dentist: Usually stress induced. See a dentist, who can adjust your bite and make a nightguard (splint). This combined with exercises for the TMJ can help. Botox injections in the muscles can also help ease the muscle tension.

Depends: Should see a dentist and possibly get to source of the grinding or have a custom mouthpiece constructed. If the teeth are worn down, this will change your bite, which in turn can cause muscle and TMJ problems.

Depends on cause: If cause is malocclusion, may need orthodontist. If slightly unbalanced occlusion, dentist may make night guard and/or do occlusal adjustment. If induced by stress, may need psychiatrist. Know that mouth guards don't "fix" your problem. They only protect your teeth from wear. Need to determine cause of the grinding and treat that in addition to the safety provided by night guard.
