50K/w X12w, 1000/day: Usual replacement schedule is 50, 000 iu per week for 12 weeks followed by daily dose of 1000 to 2000 iu/day. Once you are on that dose i would repeat vit d level to ensure normal levels.

About 5000IU/day: I usually shoot for a level of >50 and <100 but 35 is certainly better than 12 formula I use mostly is 1000iu/25# body wt it is good that you are checking this should recheck in ~ 3 months much info at vitamindcouncil.Org.
