Emergency room: If chest pain is coming from blocked artery, the time to get attention is now, in order to limit damage to the heart. Many other things can cause pain, and if the first assessment says the heart is not in immediate danger, there is time to assess more possibilities. Most worrisome symptoms often increase with exertion.

Stent: You know you have coronary artery disease and an LAD already with a stent in it. Chest pain and nausea in a patient with that package has an increased chance of being serious compared to a person without disease. You should get yourself checked out as soon as possible.
