Removal: Of gallbladder is usually recommended, because the gallbladder itself is diseased. Size of stones doesn't make any difference in case of gallstones, unlike kidney stones. This is because kidney stones, when they pass successfully, go outside the body via the bladder. Gallstones, when they pass, so into the common duct, where they cause more problems than they would have in the gallbladder.
