Diet and exercise: It sounds like you have a family history for diabetes and htn. It is likely that you are prone to or already have Insulin resistance. This problem causes diabetes, htn, lipid abnormalities. The best treatment is keeping your weight down with a low carbohydrate/starch diet, and doing as much exercise as possible.

Lifestyle change: Some parts of family history you will never be able to escape, but eating less salt, exercising (even w/o weight loss), weight loss, getting enough sleep/water, choosing wide variety of higher fiber foods and less simple sugar/high calorie foods (should be able to supply vitamins needed), all make an impact in your tendency to develop type ii diabetes, high blood pressure and vascular disease.
