Scrambled thoughts: Disorganized, scrambled thoughts with decreased ability to focus occurs with a lot of condition. Most commonly with simple fatigue. Many sedative medications and alcohol can do this. Chronic anxiety and depression also. More uncommon mental health problems like bipolar disease, schizophrenia, ptsd, etc too. Medical causes of severe fatigue include thyroid problems, drug effects, and cancer.

Many things: Is this a chronic or recent issue? This could be due to attention deficit disorder but also due to schizophrenia-like conditions or being manic. It could be caused by drugs (caffeine, amphetamines, lsd, ketamine, various pharmaceuticals). Could be related to aluminum, mercury, lead or other metals & environmental toxins or due to lyme disease, viral meningitis etc. See a good dr. For evaluation.
