Unpredictable.: A small subset of people with acute appendicitis may respond to antibiotics alone; however, it is more common for people to progress to rupture if not operated on. Therefore, for almost all people with appendicitis, the risk of rupture outweighs the risk of surgery. Therefore, we almost always recommend surgery.

No: Once the appendix gets sick as in appendicitis it will get steadily worse and eventually rupture. The time course from the onset of symptoms to rupture can vary widely based on age, sex, and even geography.
