I had mild pain in right portion of abdomen and usg report shows appendix of 6.4 mm diameter. any cure without surgery at this stage ?

Not sure: It's unclear that this mild pain is due to your appendix. But if all are convinced that you have some degree of appendicitis, then surgical removal is the usual treatment. Now, there are some studies trying to treat this problem with antibiotics, but this approach is not the standard of care. Good luck.
