Yes, but rare: Any invasive procedure can cause problems with anesthesia, the procedure itself, or infection. Allergic reaction to a medication, abnormal bleeding that becomes severe, infection with a very aggressive organism are all rare but do happen. Technical problems with the hernia repair are more likely in high risk patients due to advanced age, diabetes, chronic illness, prior repair, obesity, etc.

Yes: Bleeding, infection, cardiovascular collapse, DVT to pe, enterotomies, neurovascular compromise .... There are many many ways to die from any surgery. That's why no surgery should ever be taken lightly.
