See below: You can have mononucleosis or ebv infection with a negative mono spot test. An ebv antibody panel will be diagnostic in these cases. A number of other illnesses can mimic mononucleosis most common are cytomegalovirus infection and toxoplasmosis. Early HIV infection can also have the same symptoms.
