Sacroiliac Joint: Up to 30% of chronic low back pain generation is caused by sacroiliac joint. In a male of your age the most common cause, to my mind, is extreme sports ; especially skateboarding ; buttock falls. Pelvic ligament injury ; instability leads to parasympathetic irritation, bladder stimulation (nocturia ; interstitial cystitis) ; colon smooth muscle contraction, i.e., ibs (see my ideas quora.Com).

Biofeedback.: Mecications can help with fms, but treatment should be multifactorial. Biofeedback helps greatly with such chronic pain disorders. Massage therapy would also help.
