CKD RPGN: Rapid decline of egfr from 68 in september to 55 in january, in a young 40 years old, suggestive of rapid progressive glomerulo-nephropathy rpgn . Needs, immidiate attention, although life threatening, but treatable. Please seek help.

Kidney Function Test: A creatinine value above 20 is typocally considered abnormal. It appears that you have had some kind of renal impairment for over a decade and your doctor is sending you to hospital to have a thorough evaluation, which will probably include a series of imaging studies, lab tests, and, possibly, a renal biopsy, to determine the underlying pathology and recommend a plan of action.
