Several options: If no recent imaging, repeat MRI is warranted to exclude recurrent herniation vs scar tissue. If surgery is not warranted, can try Lyrica (pregabalin) or Gabapentin for neuropathic pain relief. Cymbalta can also be effective for chronic neuropathic pain.

Pain After Surgery: This pain in the distribution as you suggested is the result of an irritated nerve or facet joints or other injury typically in the lumbar spine (low back) requiring further evaluation by a pain specialist and may be candidate for facet injections/radiofrequency ablation, epidural steroid injection or spinal cord stimulation.
