Many mechanisms: Usually torsional, axial loading of the knee. These mechanisms occur repetitively in many sports, such as tennis, volleyball, basketball, etc. Sometimes players will continue thru the injury and not realize it. Degenerative tears can also occur, where the meniscus tears under relatively normal loading, such as in squatting or kneeling. Degenerative tears occur as we age from wear and tear.

Depends...: Most often the meniscus is torn via a twisting injury, which more often is required in "younger" people whose menisci are healthy. However, if there is osteoarthritis present, it may not take much to cause a tear, and most people with meniscal tears in the setting of arthritis do not always recall a specific injury.

Meniscal tear: Depending on your age the tear can occur traumatically and/or from "wear and tear." an acute acl tear may have an associated lateral meniscal tear; a chronic acl tear may involve a torn medial meniscus. A twisting turning motion may result in a younger patient may cause a bucket handle meniscal tear; an older patient may be susceptible to a "degenerative" tear with a horizonatl cleavage component.

Usually twisting: Younger pts will usually describe a twisting or pivoting type injury. As we get older, the meniscus loses strength and can tear with seemingly routine activities such as getting up from a chair and making a left turn.
