Dental abscess: Most very painful toothaches are of endodontic origin, not gum or perio in origin. My feeling is you need a root canal. You need to be on an antibiotic and get seen asap!

Depends: While you probably have an abscess of some kind, the origin of the infection can be either gum or tooth. Generally if the swelling is closer to where the gum and tooth meet, it's periodontal in origin. If it's further away from where the gum and tooth meet it's probably a tooth (endodontic) infection. You do need to see a dentist for an x ray, diagnosis and treatment.
