Itching skin: Itching is likely related to severe uremia that is due to her significant kidney problem. Bring her to her doctor. Need preparation for dialysis soon if uremia continues to build up. Anti histamin like Hydroxyzine may help the itchiness. Bring her to see her doctor.

Control phosphorus: Itching can be due to high serum phosphorus from kidney failure or from uremia due to build up of toxins from kidney failure. To treat high serum phosphorus, would restrict diets high is phosphorus like dairy products and take medications called phosphate binders first.

Itching and dialysis: Your mother needs to be seen by a nephrologist to see if she needs to be on dialysis. Itching is one of the indications to start dialysis. I would imagine that your mother is near 60. Have her doctor collect a 24 hour urine for creatinine clearance (cc). If the result is < 10 ml/min, she will need to start dialysis and that would stop her itching. Check her serum phosphorus and pth. Good luck.
