Not sure: Since there are no major studies on it , difficult to answer , but as with any injection , risk of infection / contamination from virum / or allergic reaction can be all side effects ; and if the doctor misses the injection point , it could cause more damage in the joint.

Pain flare up: Immediate side effects (if we can call it that way) include possible flare up of pain which usually last a couple of days. Some patients don't even experience any flare ups. See this video where i explain what is prolotherapy. http://otwithmd.com/what-is-prolotherapy/.
