Infection: You have the signs or an abscess or infection and it can spread to the rest of your body if not treated. You need to be seen and many dentists offer sedation or even sedative to take before coming in to help with anxiety. What you have needs to be treated as soon as possible though at least with medication to control spread of infection.

Sorry to hear..: It sounds like you need to find a compassionate dentist who can help you with your teeth. If not you may end up loosing all your teeth or end up in the emergency room with a serious infection if it i left untreated. You might consider asking around or calling a specialist in your area for the kind of dentist you need. Many dentist can also sedate you... Best wishes!

Must see a dentist: Find someone who is willing to sedate you if you are that fearful. You cannot leave that tooth untreated as a dental infection can spread to your heart, lungs or brain and can become life threatening. Chances are that the dentist will refer you to an oral surgeon for an extraction which can be done under IV sedation. The sooner you deal with this the sooner you will feel better.
