Phlebologists: I would advise evaluation by a phlebologist. These doctors specialize in conditions of the veins. They are called phlebologists. They are typically better equipped to treat swelling, varicosities and other conditions with small in office types of procedures.

See below: There are many treatments for spider veins such as laser or sclerosing injections. And yes if large enough they can cause pain.

Spider vein Rx.: There is no connection between spider veins as a cause of foot pain. However, treating spider veins of the foot with sclerotherapy can potentially cause foot pain if the solution injected irritates a nerve in the foot or escapes from the injected vein and irritates the surrounding skin. Because the foot has only skin and bone, sclerotherapy of the foot is usually more uncomfortable than elsewhere
