Internist or Surgeon: I would see if it resolves after treating with antibiotics prescribed by an internist or infectious disease specialist. You need to take antibiotics that cover staph.Aureus both methicillin sensitive and resistant strains as well as covering strep. Pyogenes. I would also soak it with hot salt water. If it doesn't resolve you will need incision and drainage by a surgeon.

Surgeon: This sounds like hidradenitis suppurativa. This is a skin disease affecting the apocrine glands in the axillary or groin regions. A surgeon can help this problem surgically. You may also want to consult with an infectious disease doctor.
